Diploma or College Transcript
Negative TB Test Results (must be current)
Resume
Certificate of Completion for DFPS Training

Before You Upload

To avoid delays, please make sure:

Your documents are clear and readable
Files are uploaded in PDF, JPG, or PNG format (unless otherwise noted on the form)
You use your full name on each document, if possible
You submit all required items in one sitting (if you can)
